• Supports and manages a defined list of patients requiring assessment and education about disease therapy/treatment
• Performs patient counseling using professional judgment
• Provides drug information to other health care professionals
• Documents and maintains records of all interactions
• Provides information on drug therapy, administration, side effects, and side-effect management
• Carries out interventions when appropriate
• Administers programs that help patients overcome medication-adherence barriers

• Requires vocational training, certifications, licensures, or equivalent experience
